ENROLMENT OF THE SPECIALIST TREATING PHYSICIANS

 

INTRODUCTION

 

The participation of the specialist treating physicians is dependent from the participation of their patients.

The decision to participate to I.B.AHC should be a process as much shared as possible by both the patient and the treating physician and it should be based on an equal and collaborating relationship regarding the definition of the best  therapeutic and assistance practices, and the best ways to actively support the research.

 

Also the specialist treating physicians willing to support the decision of their patients to participate to I.B.AHC, must first of all get enrolled.

 

 

ENROLMENT OF THE SPECIALIST TREATING PHYSICIANS

 

The enrolment of the treating physicians is composed by the two following steps:

  1. the treating physician signs the Informed Consent Form to authorize A.I.S.EA Onlus to the processing of his/her personal data
  2. A.I.S.EA assigns a CID  to the physician; the CID is  the anonymous identification code by which physician will be identified, in close relation with the CID of his/her patient, in the I.B.AHC Clinical Registry. In this way, also the privacy of the physician will be protected. 

A.I.S.EA Onlus will communicate the CID to the physician and also his/her account (username and password)  to login to the Registry through this portal and enter the data of his/her patient(s).

 

 

INFORMED CONSENT FORM

 

The Informed Consent Form for the treating physicians is available in the documentation area of this I.B.AHC portal, together with the Informative Letter, the Personal Data Form, and the Filling Instructions.

 

To have access to all these documents, it's necessary to login to this portal; in case of first login, the registration is required.

 

After having downloaded and read all the documents, the Consent Forms must be signed and sent to the address of A.I.S.EA Onlus.
